- Extract from the Unified State Register of Legal Entities and Individual Entrepreneurs with a date of issue no later than 30 days from the date of conclusion of the Agreement and/or relevant additional agreements to this Agreement.
In case of failure to provide an Extract from the Unified State Register of Enterprises and Organizations of Ukraine before signing - it is provided on the date of conclusion of the Agreement and/or relevant additional agreements to this Agreement;
- Charter (Description of the registrar on acceptance for registration of a new version of the Charter) or Extract from the Charter of the enterprise, which indicates the powers of the manager and founders, including restrictions on the manager to sign documents.
In case of restrictions on the manager to sign documents related to this Agreement, provide a copy of the protocol/decision/power of attorney or other document that allows signing the necessary documents;
- Certificate on the area of land plots (issued by the regional (district) administration state land agencies), owned or used (rented, subleased), certified by the signature of the manager and the seal of the manufacturer or Appendix 2 to the
Tax declaration of the single tax payer of the fourth group (if the Supplier is a single tax payer of the fourth group) or an information extract from the State Register of Real Rights to Real Estate, certified by the signature of the manager and the seal of the manufacturer (if the Supplier is a payer of income tax on general terms).
- Form 4-сг is approved by the Order of the State Statistics Service of Ukraine dated 06.07.2018 No. 133 on Sown areas of agricultural crops, if the crops, the supply of which is the subject of this contract, are subject to delivery by 02.12 of the current year, certified by the signature of the manager and the seal of the Supplier - for producers of agricultural products; and form 29-сг approved by the Order of the State Statistics Service of Ukraine dated 20.01.2020 No. 52 on the Results of the collection of agricultural crops, fruits, berries and grapes, if the crops, the supply of which is the subject of this contract, are subject to delivery after 05.01 of the following year, certified by the signature of the manager and the seal of the Supplier - for producers of agricultural products;
- A copy of the Extract from the Unified State Register of Enterprises and Organizations of Ukraine (if available);
- A copy of the certificate of registration of a value-added tax payer or an extract from the register of registration of a value-added tax payer;
- Order on the assumption of duties of the manager and the protocol on his appointment;
- Financial statements for 2019-2020 (balance!);
- Form 20-OPP with listed land plots, transport, real estate objects with a note from the State Tax Service authorities about its acceptance;
- A copy of the manager’s passport for signing documents;
- The details of the enterprise are certified by the signature and seal of the manager, and in Word format.
